HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our commitment
Titan Medical Center LLC, doing business as Titan Medical Center, is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- For treatment. We share your information with the licensed provider caring for you and with the pharmacy that dispenses any prescribed medication.
- For payment. We use limited information to process payment for medication after treatment is approved.
- For health care operations. We may use information to support the quality and administration of our services, under appropriate safeguards.
- As required by law. We disclose information when federal or Florida law requires it.
Uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke.
Your rights
- To inspect and request a copy of your health information.
- To request a correction of your health information.
- To request restrictions on certain uses and disclosures.
- To request confidential communications.
- To receive an accounting of certain disclosures.
- To receive a paper copy of this notice on request.
Record retention
We retain patient medical records for a minimum of seven (7) years from the date of last treatment, in compliance with applicable federal and state law. (Fla. Admin. Code r. 64B8-10.002)
Reporting side effects
Report suspected side effects to the FDA's MedWatch program at 1-800-FDA-1088 or fda.gov/medwatch.
Complaints and contact
If you believe your privacy rights have been violated, call us at (727) 389-3220. You may also file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
